OUR MISSION...
The Omro Hoops Club’s mission is to teach, develop, organize, manage and offer youth players the opportunity
to learn the fundamental skills of basketball while instilling life-lessons and values such as character, team work, discipline, respect, sportsmanship and developing a competitive basketball program.
2023-2024 CONSENT AND ACKNOWLEDGEMENT OF RISK
IN CONSIDERATION of the right to attend and participate in Omro Hoops Club activities,
the participant (a minor, and his parent or legal guardian) hereby:

Agrees to abide by all rules and regulations established by the Omro Hoops Club.

Acknowledges that there is an element of risk involved in any activity; certifies that the participant is physically, mentally, and emotionally capable of attending and participating in the activities; assumes all risk of a financial responsibility for any loss or injury to the participant or others that may occur as a result of the participant’s conduct; and indemnifies and holds the Omro Hoops Club harmless costs, claims, demands, charges, liabilities, obligations, judgments, executions, costs of suit and actual attorneys’ fees incurred or suffered by the Omro Hoops Club as a result of or arising out of, the participant’s conduct.

Authorizes the Omro Hoops Club or any of its agents to provide, obtain, or authorize any reasonable incidental and/or emergency medical treatment for the participant, in the event of the participant’s illness, injury, or incapacity, and hereby accepts the responsibility to pay for such treatment.

This authorization extends to any hospital or physician office and both physician and nursing personnel within the hospital or physician office, medical authorities and physicians for performing medical procedures acting on the authority of this medical treatment consent form which are deemed necessary to the aforementioned minor.
